The Priority Group System (Simplified)
The VA assigns every enrolled veteran to one of 8 priority groups. Your group determines what you pay (or don't pay) for care. Here's the breakdown:

| 1 | 50%+ service-connected disability, or unemployable due to SC condition | No copays |
| 2 | 30–40% service-connected disability | No copays |
| 3 | 10–20% SC disability, former POW, Purple Heart, discharge for disability | No copays |

| 5 | Low income (below VA means test threshold), non-service-connected | Low or no copays |
| 6 | 0% SC disability (compensable), Gulf War / PACT Act exposure, Camp Lejeune | Low copays for non-SC care |
| 7 | Income above VA threshold but below geographic threshold | Copays for most services |
| 8 | Income above geographic threshold, agrees to copay | Full copays |
Sounds straightforward, right? Here's what they don't tell you: Your priority group is assigned when you enroll—not when you need care. If you wait until you're sick or injured to enroll, you might get stuck in a higher group (meaning higher costs) even if your income has dropped.
What's NOT Obvious About VA Healthcare
- Community Care: If the VA can't see you within 30 days or you live more than 40 miles from a VA facility, you can get care from civilian providers—but only if you know how to request it
- Prescription Copays: Vary wildly by priority group and medication tier. Some veterans pay $0, others pay $11 per 30-day supply
- Dental Coverage: Extremely limited unless you're 100% P&T or have a service-connected dental condition. Most veterans need private dental insurance
- Mental Health Services: Free for most enrolled veterans regardless of priority group—a benefit many veterans don't know they have

But if you develop a service-connected condition later and get disability compensation, you'll move up to a lower group (better benefits, lower costs). Waiting to enroll could cost you thousands.

Getting enrolled is step one. Knowing which benefits to use—and when—is where most veterans leave money on the table.
For example: Did you know you can use VA healthcare for routine checkups while keeping your private insurance for specialists?
